The Rice Diet will be administered and medically supervised by Dr. Douglas Sprung, as an extension of the dietary management of obesity and other medical illnesses, provided through the Gastroenterology Group.

Please fill out these forms to be submitted upon your arrival to the Rice Diet Center. You may type directly on these PDF forms and may save a copy to your computer for most of them, although some forms will still need to be signed by hand. Please print the form after you have filled it out and bring a copy to the Rice Diet Center. Feel free to print an additional copy for your personal records.
